Job summary

Bridge Home Health & Hospice seeks a Speech-Language Pathologist to implement care standards for speech-language pathology services in home health settings and ensure adherence to the service agreement.

You will conduct evaluations, create care plans, administer therapy with specialized techniques, and coordinate with physicians to enable comprehensive patient care. Strong communication skills are essential, with ongoing education for patients and families.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology (SLP)
• Licensed as a speech-language pathologist by the State where services are furnished
• CPR certification maintained
• California driver’s license in good standing
• Minimum 1 year of clinical SLP experience
• Demonstrates strong verbal and written communication and organization skills
• Successful completion of a national SLP examination
• 350 clock hours of supervised practicum (or in process)
• At least 9 months of supervised full-time SLP services after degree

Responsibilities

• Conducts evaluations and assesses home environment for SLP needs.
• Identifies functional speech defects and develops an actionable plan of care.
• Administers speech therapy using specialized techniques/equipment.
• Recommends communication devices/aids as indicated.
• Coordinates with referring physicians to ensure coordinated care.
• Participates in case conferences and team meetings as required.
• Prepares and submits notes per policy.
• Maintains clinical competency in SLP practice.
• Identifies needs for other services and makes referrals.
• Educates patient and family/caregivers in treatment plan.
• May supervise home health aide in related therapy tasks.
• Performs other duties to facilitate SLP service delivery.
