Job summary

Bridge Home Health & Hospice is looking for a Speech-Language Pathologist to implement standards of care for SLP services and ensure adherence to the service agreement.

The role involves evaluating patients at home, developing plans of care, administering therapy, and coordinating with referring physicians to deliver comprehensive care. A master’s or doctorate in SLP, CA licensure or eligibility, and CPR certification are required.

Qualifications

• Master's or doctoral degree in SLP and state licensure or eligibility.
• 350 clock hours of supervised clinical practicum (or in progress).
• At least 9 months of supervised full-time SLP services after degree.
• Passed an approved national examination in SLP.
• Minimum 1 year of clinical SLP experience.
• Valid CPR certification.

Responsibilities

• Conduct evaluations and assess home environment for speech-language needs.
• Identify speech defects and plan care to improve function.
• Administer speech therapy using specialized techniques/equipment.
• Recommend communication devices or aids as indicated.
• Coordinate with physicians and organization staff for integrated care.
• Prepare and submit notes per policy and maintain competency.
