Job summary

Bridge Home Health & Hospice is seeking a Speech-Language Pathologist to implement standards of care for SLP services and to ensure adherence to the service agreement. The role emphasizes evaluating patients in home environments, developing care plans, and delivering therapy with appropriate devices and techniques.

The candidate will work closely with physicians and staff to coordinate comprehensive care, maintain documentation, and supervise related personnel as needed.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology (SLP) is required or must be in process of meeting licensure requirements.
• Possesses and maintains current CPR certification.
• CA driver’s license with insured vehicle as required by organization.

Responsibilities

• Conduct evaluations and assess home environment; identify equipment needs for speech/language function.
• Identify functional speech defects and establish a plan of care to improve patient’s function.
• Administer speech therapy program using specialized techniques/equipment.
• Recommend communication devices/aids as indicated.
• Confers with referring physician and staff to ensure coordinated care.
• Participate in case conferences and meetings to ensure comprehensive care.
• Prepare and submit notes per organization policy.
• Maintain clinical competency in SLP practice and theory.
• Identify patient and family needs for other services and refer as appropriate.
• Instruct patient and family in treatment regime; may supervise home health aide.
• Performs other duties to facilitate service delivery.
