Qualifications

• CPR certification and safe driving are required.

Responsibilities

• Conduct evaluations and assess home environment as needed.
• Identify functional speech defects and plan care to improve function.
• Administer speech therapy using appropriate techniques and equipment.
• Recommend communication devices as indicated.
• Coordinate with referring physicians and care team for a total plan of care.
• Participate in case conferences and team trainings as required.
• Prepare and submit progress notes per policy.
• Maintain clinical competency in SLP practice.
• Identify needs for other services and refer as appropriate.
• Instruct patient and family in treatment regimen as indicated.
• May supervise home health aides in related procedures.
• Perform other duties to deliver SLP services.
