Job summary

Bridge Home Health & Hospice is seeking a Speech-Language Pathologist to implement standards of care for SLP services in a home health setting in California. The role will involve evaluating patients, planning care, and delivering therapy using specialized techniques and equipment.

The ideal candidate has a Master’s or Doctoral degree in SLP, CA licensure (or equivalent), and current CPR certification. Strong communication and organizational skills are essential for coordinating with physicians

Qualifications

• Master's or doctoral degree in speech-language pathology (SLP) as required.
• Licensed as a speech-language pathologist by the State where service is provided.
• CPR certification is current and maintained.

Responsibilities

• Conducts evaluations and assesses home environment for speech-language needs.
• Identifies defects and plans care to improve function.
• Administers speech therapy using specialized techniques or equipment.
• Recommends communication devices as indicated.
• Collaborates with referring physician and care team for coordinated care.
• Participates in case conferences and staff trainings as needed.
• Documents notes per policy and maintains clinical competency.
• Identifies needs for other services and makes referrals as appropriate.
• Instructs patient and caregivers in treatment plan and procedures.
• Supervises home health aide as needed in SLP-related tasks.
• Performs other duties to facilitate SLP service delivery.
