21st Century Home Health Services (21HHS) is seeking a Home Health Speech Therapist to join a large Bay Area-based team. The role partners with case managers to deliver skilled services for breathing, speaking and swallowing, while promoting education and patient advocacy within an interdisciplinary care team.

Qualified candidates hold a CA Speech Therapist license and BLS/CPR, with a valid CA driver's license. Comprehensive benefits and strong growth opportunities are offered.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License.
• Knowledge of development and human behavior as it relates to illness/injury and culture in health care.
• Knowledge of state and federal home-care regulatory guidelines preferred.
• Experience in home health care highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team highly desired.

What you’ll do as a Home Health Speech Therapist/Language Pathology (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.
