Responsibilities

What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST)

• In partnership with the Case Manager, provide skilled services to assist patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive, and fluency disorders that affect communication.
• Serve as an educator, patient advocate, and member of the interdisciplinary care team.

Qualifications

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of individual development and human behavior as it relates to illness/injury and the influence of culture on health care
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Experience in home health care (highly preferred)
• Experience with high-acuity patients (a plus)
• Experience working with an interdisciplinary team (highly desired)
• Case Management fundamentals (highly desired)
• Self-directed, empathetic, critical thinker with outstanding interpersonal and organizational skills
• Leadership skills

Pre-interview requirement

Compensation

$110,000 - $125,000 per year

Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (up to 4% match).
