Job summary

21st Century Home Health Services (21HHS) seeks a Speech Therapist to provide in-home services to patients across California. You will evaluate and treat speech, language, cognitive, and swallowing disorders in collaboration with the Case Manager and interdisciplinary team.

The role includes flexible scheduling, small coverage areas, autonomy, and strong emphasis on patient advocacy, education, and quality outcomes within a leading home health agency.

Qualifications

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience in home health care is preferred

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.

• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
