21st Century Home Health Services (21HHS) is seeking a Home Health Speech Therapist/Language Pathologist to serve patients in San Mateo County, including South San Francisco and Pacifica. The role involves evaluating and treating communication disorders, partnering with Case Managers, and acting as an educator and advocate within a multidisciplinary team.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR certification.
• Valid CA Driver’s License.
• Knowledge of development and behavior related to illness/injury; culture in health care.
• Knowledge of state and federal home-care regulatory guidelines preferred.
• Home health care experience preferred.
• Experience with high-acuity patients a plus.
• Experience with an interdisciplinary team desired.
• Case Management fundamentals desired.
• Self-directed and empathetic with strong interpersonal skills.

We care for over 4,000 patients across San Francisco and surrounding counties, and we are expanding into Marin and Sonoma counties.

Prerequisites for all positions at 21HHS: All opportunities require being in the field visiting patients in their homes. Remote/work‑from‑home positions are not available unless specifically noted.

What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST):
