Job summary

Community Bridges, Inc. is seeking a Specialty Case Manager to provide community-based case management for patients with serious mental illness or high-risk needs.

You will assist in planning, advocacy, coordination and monitoring to access health services, housing, employment, and social supports. Responsibilities include assessing needs, developing treatment plans, documenting services, and applying recovery-oriented practices to support clients in achieving stable, safe lives.

Qualifications

• High school diploma or GED required; associates degree or higher preferred in behavioral health.
• 1 year of behavioral health outreach/engagement experience preferred
• Current AZ Driver's License required
• Arizona Fingerprint Clearance card required
• Jail Clearance if applicable
• Behavioral Health Technician (BHT) certification per policy
• Peer certification where applicable

Responsibilities

• Provide community-based case management for patients with serious mental illness or high-risk needs.
• Assist in planning, advocacy, coordination and monitoring to access health services, housing, employment and supports.
• Assess needs, develop and monitor individualized treatment plans, document services.
• Apply recovery-oriented practices to promote safe, recovery-focused outcomes for clients.
