The Familiar Face Case Manager at Community Bridges, Inc. is a behavioral health professional who specializes in care coordination and discharge planning for individuals utilizing crisis services with high utilization rates.

The role focuses on preparing patients for appropriate services, helping obtain those services, coordinating with all parties involved, and reducing barriers to treatment. Reporting to the Clinical Director, the position requires a high school diploma or GED (associates

Qualifications

• Highschool diploma or GED required.
• Associates Degree in a field related to behavioral health preferred.
• Case Management experience preferred.
• Minimum of six (6) months of experience in multiple settings and levels of care (mental health and substance use) required.
• One (1) or more years of experience preferred.
• Fingerprints Clearance Card required.
• Behavioral Health Technician (BHT) certification per policy.

Responsibilities

• Coordinate care and discharge planning for crisis-service users.
• Prepare patients for clinically appropriate services and assist in obtaining those services.
• Coordinate care with all involved parties and reduce barriers to treatment.
