Job summary

The Bridge Bronx, NY is seeking Scattered Site Case Managers to support formerly homeless adults with serious mental illness and/or substance use disorders. Clients live in apartments and receive community-based services, including assessments and Support Plans in electronic records.

You will link clients to medical and mental health care, coordinate with providers, facilitate groups, document timely care, and respond to crises using motivational interviewing and culturally competent approaches.

Qualifications

• Bachelor's Degree required.
• Experience working with mental health, crisis intervention, and trauma-triggered behavior.
• Excellent time management and organizational skills.
• Team player who is stress resistant, flexible, creative, and open to feedback.
• Willingness to perform CPR/First Aid/AED/ Naloxone.
• Preferred knowledge of AWARDS.
• Bilingual in Spanish is a plus.

Responsibilities

• Provide case management and interventions to high-risk clients, including assessments and referrals.
• Create and update Support Plans in the electronic client records system.
• Utilize Motivational Interviewing techniques to support housing stability.
• Coordinate care with medical, mental health, and social service providers.
• Co-facilitate groups on wellness, housing, stability, employment and education, and relationships.
• Complete required documentation for all client contacts in accordance with policy.
