Job summary

Community Bridges, Inc. is seeking a Specialty Case Manager to provide in-community case management for individuals with mental health or substance use diagnoses, survivors of domestic violence, and high‑risk clients.

You will plan, connect, advocate, coordinate, and monitor services to access health care, housing, employment, education, and supports, while documenting accurately. The role requires a high school diploma or GED, 1–3 years in behavioral health, and a valid AZ driver’s license;

Qualifications

• High school diploma or GED required.
• Associate’s degree or higher in behavioral health preferred.
• 1–3 years of experience in a behavioral health position providing outreach and engagement.
• 1–3 years of experience with special populations (SMI, DV survivors, high-need clients) preferred.
• Or equivalent combination of education and experience.
• Valid Arizona driver’s license in good standing.
• Arizona 39‑month MVR.
• Arizona Fingerprint Clearance Card.
• Jail clearance (if applicable).

Responsibilities

• Provide case management services in the community to individuals with mental health/substance use diagnoses.
• Plan, connect, advocate, coordinate, and monitor services for access to health care, housing, employment, education and supports.
• Assess needs, develop and monitor individualized treatment plans, document services timely and accurately.
• Promote recovery and a safe recovery environment for high‑risk clients using advanced techniques.
