Job summary

Community Bridges, Inc. (CBI) is seeking a Specialty Case Manager to deliver community-based case management for individuals with mental health and/or substance use concerns.

You will plan, connect, advocate, coordinate, and monitor access to health care, housing, and social services for high-risk clients and survivors of domestic violence. You will assess needs, develop individualized treatment plans, and document services accurately.

Qualifications

  • High school diploma or GED required; associate’s degree or higher in a field related to behavioral health preferred.
  • Recovery from alcohol or drugs and/or general mental health for equal to or greater than one consecutive year preferred.
  • 1‑3 years of experience in a behavioral health position providing outreach and engagement activities preferred.
  • 1‑3 years of experience working with special populations (e.g., serious mental illness, domestic violence survivors, high-cost/high-need clients) preferred.
  • Or equivalent combination of education and experience.
  • Current Arizona driver’s license, valid and in good standing, and 39‑month MVR.
  • Peer certification (if applicable).
  • Arizona fingerprint clearance card.
  • Jail clearance (if applicable).

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs across Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Specialty Case Manager

This role provides case management services in the community to individuals with mental health and/or substance use diagnoses, survivors of domestic violence, and high-risk clients. Responsibilities include planning, connecting, advocating, coordinating, and monitoring services to help patients access health care, financial assistance, housing, employment, education, social services, and natural supports.
