The Case Manager will document client progress, maintain regulatory compliance, participate in treatment teams, and may perform routine toxicology

Qualifications

• High school diploma or equivalent required; Bachelors preferred for related field.
• AA degree with related experience acceptable in lieu of a degree.
• Certification as a substance use counselor is preferred where applicable.

Responsibilities

• Provide case management, care coordination, and recovery support to clients.
• Develop individualized care plans with clients and treatment team.
• Assist with housing navigation, benefits, and community reintegration.
• Coordinate with medical and social service partners; ensure proper documentation.
• Complete progress notes, intake/discharge paperwork, and required reports.
