Community Bridges, Inc. is seeking a Specialty Case Manager to provide in-community case management for individuals with mental health or substance use diagnoses, survivors of domestic violence, and high‑risk clients.
You will plan, connect, advocate, coordinate, and monitor services to access health care, housing, employment, education, and supports, while documenting accurately. The role requires a high school diploma or GED, 1–3 years in behavioral health, and a valid AZ driver’s license;
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
The Specialty Case Manager provides case management services in the community to individuals who have a mental health and/or substance use diagnosis, survivors of domestic violence, and/or those considered high‑risk due to their acuity level. The role includes planning, connecting, advocacy, coordination, and monitoring to help patients gain access to health services, financial assistance, housing, employment, education, social services, and natural supports. The manager assesses needs, develops and monitors individualized treatment plans, and documents services accurately and timely. The Specialty Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients while implementing advanced techniques and interventions for high‑risk individuals.
