Community Bridges, Inc. (CBI) is seeking a Case Manager to provide comprehensive case management in community or office settings, coordinating services and supporting patients and families.

You will assist clients in accessing health services, financial aid, employment, education, social supports, and natural supports while developing tailored treatment plans and ensuring timely documentation. The role embraces a recovery-focused approach and a safe, supportive environment.

Qualifications

• Highschool diploma or GED required; associates degree or higher in behavioral health preferred.
• 6-month recovery from substance use and/or mental health preferred.
• 1–3 years GMH/SU population experience required.
• 1–3 years outreach and engagement in behavioral health required.
• 1–3 years medical setting experience preferred.
• Valid identification required.
• AZ Driver's License preferred.
• 39-month MVR preferred.
• Arizona Fingerprint Clearance card required by policy.

Responsibilities

• The Case Manager provides case management in the community or office settings.
• Assist patients and care teams in planning, connecting, advocating, coordinating and monitoring needs.
• Help patients access health services, financial assistance, employment, education, and social supports.
• Assess needs, develop and monitor individualized treatment plans, document services accurately.
• Uphold recovery philosophy and promote a safe recovery environment for clients.
