Job summary

Community Bridges, Inc. (CBI) is seeking a Specialty Case Manager to deliver community-based case management for individuals with mental health and/or substance use concerns.

You will plan, connect, advocate, coordinate, and monitor access to health care, housing, and social services for high-risk clients and survivors of domestic violence. You will assess needs, develop individualized treatment plans, and document services accurately.

Qualifications

• High school diploma or GED required; associate’s degree or higher in a field related to behavioral health preferred.
• Recovery from alcohol or drugs and/or general mental health for equal to or greater than one consecutive year preferred.
• 1‑3 years of experience in a behavioral health position providing outreach and engagement activities preferred.
• 1‑3 years of experience working with special populations (e.g., serious mental illness, domestic violence survivors, high-cost/high-need clients) preferred.
• Or equivalent combination of education and experience.
• Current Arizona driver’s license, valid and in good standing, and 39‑month MVR.
• Peer certification (if applicable).
• Arizona fingerprint clearance card.
• Jail clearance (if applicable).

Responsibilities

• Plan, connect, advocate, coordinate, and monitor services for patients with mental health/substance use diagnoses, domestic violence survivors, and high-risk clients.
• Assist patients in accessing health services, financial assistance, housing, employment, education, social services, and natural supports.
• Assess client needs and develop individualized treatment plans.
• Document services and ensure accurate, timely record-keeping.
• Implement advanced techniques and interventions tailored to high-risk individuals.
