Daymark Recovery Services is seeking a Care Manager to assess, plan, implement, and monitor care for individuals with complex behavioral health and medical needs in North Carolina. You will coordinate referrals, document plans, and work with a multidisciplinary team to ensure timely interventions and safe health outcomes.

The role emphasizes person-centered care, data-driven planning, and collaboration with clinicians, community partners, and families to support recovery and continuity of care.

Qualifications

• Associates or Bachelor’s in a human service field is preferred.
• RN with MH/SA/DD experience may substitute degree requirements.
• Masters degree with licensure is valued.
• Non-human-service Bachelors with MH/SA/DD experience accepted.

Responsibilities

• Conduct care management assessments and develop Care Management Plans.
• Coordinate referrals and linkage to needed services and supports.
• Monitor and follow up with client and referrals to ensure plan effectiveness.
• Participate in interdisciplinary treatment planning and team consultations.
• Provide crisis intervention when needed and coordinate with crisis services.
• Document needs, goals, and progress in consumer records and ensure proper authorization forms.
• Assist with provider coordination and ensure continuity of care across levels.
