Daymark Recovery Services, Inc. Greensboro, NC is hiring a Care Management Extender to provide care management functions under supervision. You will refer and link clients to needed services and monitor follow-up with the consumer and referrals.

The role supports crisis intervention when needed, participates in treatment planning, and ensures all parties are informed of the plan of care. Strong documentation and coordination with providers are essential.

Qualifications

• Minimum of a high school diploma or equivalent and meet one of the listed criteria.
• Certified Peer Support Specialist, CHW, or related lived experience criteria acceptable.
• Experience or education aligning with care management and health services.

Responsabilités

• Provide care management extender duties, referring and linking to needed services and monitoring follow-up.
• Participate in interdisciplinary treatment planning and communicate the plan of care to involved parties.
• Provide crisis intervention and involve crisis services when needed.
• Perform other duties as assigned by supervisor.
• Coordinate care management documentation with family, providers, and consumer records.
