Job summary

Daymark Recovery Services is seeking a Care Management Extender to provide care management, linkage to services, and follow‑up for individuals with complex behavioral health, medical, and psychosocial needs. The role requires coordinating with providers and community partners to ensure timely interventions and safety.

Qualifications

• High school diploma or equivalent required.
• Experience with 1915i, adult placement, emergent housing support, personal care services and long term care preferred.

Responsibilities

• Provide care management extender duties and link to needed services and education for health promotion.
• Monitor follow‑up with clients and referrals to ensure plan of care is implemented.
• Provide crisis intervention to all participants and involve crisis services when needed.
• Coordinate with interdisciplinary treatment planning and ensure all parties are aware of the plan of care.
• Collect and document authorization to disclose health information prior to releasing any information when needed.
