Job summary

Daymark Recovery Services seeks a care manager to perform comprehensive assessments, develop person-centered plans, and link clients to needed services in a therapeutic setting. You will monitor care, coordinate with an interdisciplinary team, and track outcomes while delivering crisis intervention when necessary.

You will collect information across medical, psychosocial, and environmental domains and collaborate with family, providers, and caregivers to create effective care plans and ensure

Qualifications

• Associates or bachelor’s degree in a human service field.
• Two years MH/SA/DD experience with the population served.
• Licensed RN with two years MH/SA/DD experience.
• Masters with licensure or a Masters in a human service field with one year MH/SA/DD experience.

Responsibilities

• Performs care management assessment and develops care plans; refers and links to needed services.
• Monitors follow-up with clients and referrals; provides education for health promotion.
• Participates in interdisciplinary treatment planning and ensures team awareness of the plan.
• Provides crisis intervention when needed and coordinates with crisis services as required.
• Coordinates care management activities to achieve identified outcomes.
