Daymark Recovery Services is seeking a Care Management Extender to support comprehensive care coordination for individuals with behavioral health needs. You will perform care management tasks, referrals, and follow-up under supervision, ensuring the plan of care is understood by clients and providers.

The role emphasizes timely referrals, education on health promotion, and active participation in treatment planning within a multidisciplinary team to support recovery goals.

Qualifications

• Minimum of a high school diploma or equivalent.
• Certified Peer Support Specialist or CHW credentials are preferred.
• NC CHW SCCT training is an asset.
• Experience in behavioral health and care coordination is valued.

Responsibilities

• Provide care management extender duties under supervision.
• Refer and link clients to medical, behavioral, social, and community services.
• Monitor and follow up with clients and referrals.
• Educate clients on health promotion and plan of care.
• Participate in interdisciplinary treatment planning and coordination.
• Provide crisis intervention when needed and engage crisis services.
