Daymark Recovery Services, Inc. is seeking a Care Manager to perform case management assessment, person-centered planning, referral/linkage, and progress monitoring in Buies Creek, NC.

The role involves coordinating services, educating clients on health promotion, and ensuring documentation compliance. Requirements include an associates or bachelor's degree in a human service field with MH/SA/DD experience, or an RN with MH/SA/DD experience, or a Master’s with licensure.

Qualifications

• Requires an associates or bachelor’s degree in a human service field with MH/SA/DD experience; or an RN with MH/SA/DD experience; or a Master’s with licensure and related experience.
• Experience in any portion of care management planning, documentation, and crisis intervention.
• Knowledge of Medicaid/NC Health Choice and care coordination best practices.

Responsibilities

• Provides care management assessment and reassessment, develops care plans, and coordinates services.
• Maintains up-to-date documentation and monitors progress toward goals.
• Provides crisis intervention and collaborates with interdisciplinary teams.
• Facilitates referrals and linkage to medical, behavioral health, and social services.
• Ensures signed authorizations and proper release of information are on file.
