Job summary

Community Bridges, Inc. (CBI) provides comprehensive behavioral health services across Arizona through residential, outpatient, inpatient and community programs.

The Specialty Case Manager conducts community-based case management for patients with serious mental illness or high-risk statuses, coordinating access to health services, housing, employment and education. The role emphasizes planning, advocacy and monitoring, with a focus on recovery-oriented care and timely documentation of services

Qualifications

• High school diploma or GED required; associates degree or higher in behavioral health preferred.
• Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year preferred.
• 1 year of experience in a behavioral health position providing outreach and engagement activities.

Responsibilities

• Provides community-based case management to patients experiencing serious mental illness or high acuity.
• Assists in planning, connecting, advocacy, coordination and monitoring to access health services, housing, employment, education and social supports.
• Develops and monitors individualized treatment plans and documents services accurately and timely.
• Promotes recovery and a safe recovery environment; uses advanced techniques to serve high-risk individuals within the program.
