The SMI Case Manager delivers community-based care, coordinates services, and supports families to access health, financial aid, employment and education, while documenting progress in a timely manner. Qualifications

• Highschool diploma or GED required.
• Associates degree or higher preferred.
• 1 year of experience in a behavioral health position.
• Minimum six months of recovery from substance use/mental health disorders preferred.
• Valid AZ Driver's License in good standing.
• Clear 39-month MVR.
• BHT certification per policy.
• Arizona Fingerprint Clearance card required.

Responsibilities

• Provide case management services in the community to patients with serious mental illness and families.
• Assist in planning, connecting, advocacy, coordination and monitoring; help patients access health services, financial assistance, employment and education.
• Develop and monitor individualized treatment plans and document services accurately and timely.
• Embrace recovery philosophy and promote a safe recovery environment for clients.
