Genesis seeks a skilled Social Services professional to join our long-term care team in a role focused on comprehensive psychosocial care for patients and residents. You will collaborate with the interdisciplinary team to safeguard patient rights and promote psychosocial well-being across the facility.

The position requires a Master's degree in social work or a related field, along with any required state certifications for practice in long-term care.

Qualificações

• Master's degree in social work or human services is required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsabilidades

• Assists with planning and implementing a comprehensive Social Services program.
• Works with the interdisciplinary team to promote and protect patients/residents' rights and psychosocial well-being.
• Complete a comprehensive Psychosocial Assessment for each patient/resident identifying social, emotional, and psychological needs and strengths; assess discharge needs.
• Identifies discharge goals at admission and documents the discharge plan with patient/family involvement.
• Educates staff regarding the role of Social Services and psychosocial needs of patients/residents and families.
