Job summary

Genesis HealthCare in the United States seeks a Social Services Specialist for a nursing center to assess social, emotional, and mental health needs of patients and residents and arrange services to improve overall well-being and quality of life.

You will collaborate with the interdisciplinary team to develop care plans, educate staff on social services, and support families. A bachelor’s degree in social work or human services and applicable state licensure are required.

Qualifications

• Bachelor's degree in social work or human services.
• State licensure/certifications as required by the State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Education

Bachelor's degree in social work or human services field
