Job summary

Genesis is seeking a Social Services Specialist for our nursing center to work with patients and families, assess social and emotional needs, and coordinate services that enhance overall well-being and quality of life.

You will collaborate with an interdisciplinary team, participate in care planning, and provide education and support to patients and families while adhering to NASW Code of Ethics and applicable regulations.

Qualifications

• Bachelor's degree in social work or human services required.
• Must possess certifcations/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient rights and well-being.
• Participate in development of an interdisciplinary plan of care for each patient.
• Hold meetings with patients, families, and significant others to provide support and information to improve quality of life.
• Perform discharge planning activities including education, documentation and referrals.
• Establish relationships with community-based agencies to benefit patients and residents.

Education

Bachelor's degree in social work or human services field
