Job summary

Genesis is seeking a Social Services Specialist at our nursing center in West Virginia. The role focuses on identifying social, emotional, and mental health needs of patients/residents and coordinating services to meet those needs, in alignment with NASW Code of Ethics and applicable laws.

You will collaborate with an interdisciplinary team, develop care plans, conduct meetings with patients and families, and handle discharge planning while maintaining strong community referrals.

Qualifications

• Bachelor's degree in social work or human services field is required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
