Job summary

Genesis in Cumberland, RI is seeking a Social Services Specialist to work with patients/residents and families to identify social, emotional, and mental health needs and connect services. You will develop interdisciplinary care plans, educate staff on social services, conduct discharge planning, and build relationships with community agencies to support quality of life.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess certifications/licensures as required by state to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of an interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform discharge planning activities including family/patient education, documentation and referrals to appropriate agencies.
• Establish relationships and maintain contact with community-based agencies/services that benefit our patient/resident population.
