Job summary

Genesis in Montgomery County, PA is seeking a licensed social services professional to collaborate with the interdisciplinary team to promote patient and resident well-being and educate staff on the social services role. You will help develop written care plans, hold meetings with patients and families, and assist with discharge planning and community referrals to enhance quality of life.

The position requires a Bachelor’s degree in social work or human services and appropriate state licensure

Qualifications

• Bachelor's degree in social work or human services required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services.
