Qualifications

• Bachelor's degree in social work or human services is required.
• Certifications/licensures as required by the State of employment to practice in long-term care are required.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, documentation and referrals.
• Establish relationships and maintain contact with community-based agencies/services for the patient/resident population.

Education

Bachelor's degree in social work or human services field
