Job summary

Genesis HealthCare is seeking a Social Services Specialist to support patients and families in a nursing center. You will identify social, emotional, and mental health needs and coordinate services to enhance quality of life and well-being.

You will work with an interdisciplinary team to develop care plans, hold meetings with families, and arrange referrals to community resources. A focus on compassionate, compliant care is essential.

Qualifications

• Bachelor's degree in social work or human services required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient rights and well-being.
• Participate in development of written interdisciplinary plan of care for each patient to address social service needs.
• Hold meetings with patients, families, and significant others to provide support and information to improve quality of life.
• Perform discharge planning activities including education, documentation and referrals to appropriate agencies.
• Establish relationships and maintain contact and referral flow with community-based agencies and services.
