Job summary

LEGACY HEALTHCARE MANAGEMENT seeks a Social Work Tech to support residents’ social, emotional, and psychological needs in a long-term care setting. You will develop and carry out plans to maximize residents’ potential during their stay and educate families about post-acute options.

The role involves discharge planning, coordinating with the interdisciplinary team, and documenting care in the medical record. Experience in social work within long-term care is preferred.

Qualifications

• 1 year of supervised social work or discharge planning experience in a long term care setting preferred.

Responsibilities

• Complete and document Social Work and Psychosocial assessments within 3 days of admission or readmission.
• Ensure 3877/78 forms are completed and accurate at admission and annually.
• Schedule 48-hour Resident Care Conference for new admissions and notify the IDT by email.
• Complete MDS sections and the appropriate CAAs.
• Develop and update Care Plans including psychosocial, discharge planning, behavioral, and cognitive aspects.
• Coordinate care with psych provider including medication management and consents.
• Coordinate ancillary services (Dental, Ophthalmology, Audiology) with input from DON/Administrator.
• Educate residents and families about Advanced Directives and complete related forms within 48 hours of admission.
• Greet residents and families, initiate visits, and document interactions in the chart.
• Demonstrate proficiency with EMR and support resident adjustment to the facility.
