Job summary

Ascension Dell Seton Medical Center in Austin, TX is seeking a dedicated Case Manager to design personalized care plans and coordinate discharge planning.

Qualifications

• Collaborate with the interdisciplinary care team to refine treatment strategies and ensure high-quality medical documentation.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to
  hire date or job transfer date required.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• Bachelor's degree in Social Work required.

Work Experience:

• 3 years of experience preferred.

What additional preferences we're seeking

• Clinical Case Management Experience: A proven track record of coordinating complex discharges and care transitions within an acute care or clinical setting
• Active State Licensure: Current and unrestricted license in Social Work (LMSW/LCSW) as required by state regulations.
