Job summary

Ascension St. Joseph's Hospital in Milwaukee, WI, seeks a part-time Case Manager in the Case Management department. You will develop patient care plans, assess complex issues, and advocate for patients to ensure their rights and preferences are respected within the healthcare system.

You will collaborate with leadership and care transition teams to ensure effective implementation and ongoing reevaluation of plans. Licensure or certification is required or pursued, with CCMC preferred.

Qualifications

• Licensure / Certification / Registration required: LCC or Social Worker credentials from Wisconsin boards or CCMC preferred.

Responsibilities

• Develop care plans for patients based on assessment findings and needs.
• Assess complex emotional, psychological and physical issues to determine support required.
• Advocate for patients' rights and coordinate with care teams for plan implementation.
• Maintain ongoing dialogue with leadership and care transition to reevaluate care plans.

How you’ll make an impact in this role

What minimum requirements you’ll need

Licensure / Certification / Registration:

• One or more of the following required:
• Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
