Ascension is seeking a dedicated Social Worker to join the Adult Inpatient team in Indianapolis, IN at Ascension St. Vincent. You will assess emotional and social barriers and coordinate with interdisciplinary teams to ensure safe transitions from admission to discharge.
The role requires a Bachelor's and a Master's in Social Work, licensure as a social worker, and experience supporting patients. Preference for CCMC/ACM credentials and 3 years of experience; strong advocacy and patient education
