Ascension in Panama City, FL seeks a Case Management professional to develop integrated care plans that respect patient and family preferences. You will evaluate needs such as legal, psychosocial, and crisis situations, and coordinate with the care transition team for effective post-discharge services.

The role emphasizes advocacy, resource navigation, and proper documentation to support complex care decisions within Ascension Sacred Heart Bay.

Formación

• High School diploma or equivalent with 2 years of cumulative experience, or an Associate's/Technical degree, or 4 years of applicable cumulative job-specific experience.
• Clinical Counselor credential or Social Worker with Clinical Practice credential from the Florida Board.

• Assesses, evaluates, recommends and treats legal, neglect, substance abuse, crisis, psychosocial and adjustment needs.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialogue with supervisor and care transition team to implement and reevaluate health plans.
• Ensures comprehensive medical record documentation and appropriate post-discharge care options.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • None Required.
• Preferred Credential(s):
  • Clinical Counselor.
  • Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling.

Education

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
