Ascension Sacred Heart Bay in Panama City, FL seeks a part-time Case Manager within the Case Management department. • Serves as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses and treatment options for post-discharge care. Acts as a liaison between patient and post acute services.
• Collaborates with payers for benefit optimization.
• Advocates for effective and efficient utilization of clinical resources.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date.
• Preferred Credential(s):
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC). ACM also accepted.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
