Job summary

Ascension Milwaukee seeks a Case Manager to assess patients’ needs and coordinate interdisciplinary care from admission to discharge. The role provides support and education on resources addressing emotional, financial, and social needs arising from a diagnosis.

You will develop care plans, assess complex issues, advocate for patient rights, and collaborate with care transition teams to implement and reevaluate plans.

Qualifications

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Your future role at a glance

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
• • One or more of the following:
  • • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
    • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Preferred Credential(s):
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
