Ascension St. Vincent's Riverside Hospital in Jacksonville, FL is seeking a qualified social work/case management professional for a Day shift, part-time role.

The position focuses on coordinating care across the continuum, integrating patient and family preferences, and ensuring effective post-discharge planning within the hospital setting. The role requires a Master’s degree in Social Work, licensure/certification in relevant clinical practice areas, and preferred CCS/CMA credentials.

Competenze

• Master's degree in Social Work required.
• 3 years of experience preferred.
• Licensure / Certification / Registration: Clinical Counselor, Counselor specializing in Clinical Practice, Social Worker specializing in Clinical Practice.
• Preferred credentials: ACMA, CCMC.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
• One or more of the following:
• Clinical Counselor obtained prior to hire date or job transfer date.
• Counselor specializing in Clinical Practice obtained prior to hire date or job transfer date.
• Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.

Preferred Credential(s):

• Case Manager credentialed from the American Case Management Association (ACMA).
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education

• Master's degree in Social Work required.

Work Experience

• 3 years of experience preferred.
