Job summary

Ascension is seeking a PRN Registered Nurse at Ascension St. Vincent's Riverside Hospital in Jacksonville, FL to coordinate comprehensive care across the patient journey. You will work in a fast-paced, multidisciplinary environment to ensure seamless care from admission to discharge.

The role emphasizes patient-centered strategies, documentation accuracy, and collaboration with physicians and teams to optimize outcomes and resource use.

Qualifications

• Registered Nurse credential from the Florida Board of Nursing prior to hire date.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

Responsibilities

• Orchestrate the full interdisciplinary plan of care from admission through discharge to ensure a seamless patient experience.
• Design personalized care strategies that integrate clinical requirements with patient and family values and preferences.
• Monitor patient progress and oversee medical documentation to ensure treatment options and services align with diagnostic needs.
• Advocate for essential resources and proactively remove barriers to ensure the effective delivery of the health plan.
• Maintain continuous collaboration with supervisors and providers to optimize care implementation and post-discharge success.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration
