Ascension St. Vincent's Roger Main Building in Jacksonville, FL is hiring a full-time Social Worker.

You’ll investigate, assess, and plan interventions to help patients cope with social, emotional, economic and environmental problems, interviewing patients and documenting case activity. You’ll develop community connections, refer to resources, support discharge planning and post-discharge needs, and collaborate with welfare and health agencies.

Formación

• Florida Board licensure preferred for social work roles.
• High school diploma or higher with relevant experience.
• Approximately 3 years of related work experience preferred.

How you’ll make an impact in this role

• Investigate, assess, and plan interventions to help patients cope with social, emotional, economic and environmental problems.
• Interview patients, assesses priorities and documents case activity.
• Assist clients/families with obtaining community assistance by referral to proper resources.
• Develop and maintain working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
• Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Social Work credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.

Education

• High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.

Work Experience

• 3 years of experience preferred.
