Qualifications

• Master's degree in Social Work (MSW).
• One year of clinical social work experience preferred.
• AHA BLS - Healthcare Provider certification required.
• Licensed Social Worker (LSW/ LCSW) preferred.
• Strong communication, organization and planning skills.

Responsibilities

• Assesses, diagnoses, intervenes with and plans for social work services related to patient and family bio-psycho-social needs and behaviors.
• Lead discharge planning and identify barriers to timely discharge.
• Refer patients to appropriate social and financial resources post discharge.
• Address abuse, neglect, domestic violence, and crisis interventions.
• Occasional weekend commitments.
