Job summary

Mercy Health is seeking a Social Worker Care Manager at Anderson Hospital to coordinate discharge planning and provide clinical supervision. You will access hospital, community, and governmental resources to support patients and families throughout the continuum of care.

Responsibilities include identifying patients needing social services, documenting plans, and collaborating with the care team to ensure goals are met.

Qualifications

• Bachelor of Social Work is required.
• Master’s degree in SW or healthcare field preferred.
• BLS Basic Life Support required by American Heart Association.
• Licensed as a Social Worker in state of practice (required, VA preferred).
• ACM or CCM certification preferred.

Responsibilities

• Identifies and prioritizes patients in need of social services with holistic approach.
• Plans with patients, caregivers and healthcare team for outcomes.
• Documents plan of care and reflects patient/family involvement.
• Follows standardized practices for Advance Care Planning and length of stay management.
• Supports denial prevention and facilitates discharge planning rounds.
• Promotes proactive care and removes barriers to testing and treatment.
