Job summary

Excel Care in Lewes, DE is seeking a compassionate Licensed Social Worker (LSW) or Certified Social Worker (CSW) to join our Social Services team on a part-time basis.

You will complete psychosocial assessments, support residents and families, coordinate discharge planning, and advocate for residents' rights while maintaining compliant documentation.
