You'll perform psychosocial assessments, assist with discharge planning, and advocate for residents' rights within a multidisciplinary team while maintaining compliant documentation and high-quality, compassionate care.

Qualifications

• Must be state-certified LSW or CSW.
• Experience in long-term care or skilled nursing preferred.
• Excellent communication and documentation skills.

Responsibilities

• Complete psychosocial assessments and participate in care planning.
• Support residents and families with adjustment, concerns, and emotional needs.
