Job summary

Excelcare-At-Lewes in Lewes, DE is hiring a part-time Licensed Social Worker (LSW/CSW) to join its Social Services team. Immediate start, 25 hours per week, and a mission to support residents and families through care transitions.

You'll perform psychosocial assessments, assist with discharge planning, and advocate for residents' rights within a multidisciplinary team while maintaining compliant documentation and high-quality, compassionate care.

Qualifications

• Must be state-certified LSW or CSW.
