Complete Care Management is seeking an experienced Social Worker (LSW/CSW/LCSW) to join our skilled nursing and rehab team in New Jersey. This part-time role focuses on admissions, discharge planning, family support, and care coordination to help residents and families through transitions.

You will provide emotional support, advocate for residents, participate in interdisciplinary care plan meetings, and ensure accurate documentation in line with state and federal regulations.

Qualifications

• Current New Jersey LSW, CSW, or LCSW license is required.
• Minimum 1–2 years in a healthcare setting, preferably skilled nursing, long-term care, or rehab.
• Experience with discharge planning, Medicaid/Medicare, and MLTS preferred.
• Understanding of CMS regulations and health department survey processes preferred.
• Strong communication, documentation, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.
